• No Form A: Maharashtra requires the state-prescribed Form A for state authorities. Plain-paper applications can be returned (though Bombay HC has held PIOs cannot reject for missing form alone).
• Wrong language for the office: most state PIOs in Mumbai accept Marathi + Hindi + English. In rural districts, Marathi-only is often slower; bilingual is fastest.
