Quick answer

  • What the login asks. Open beneficiary.nha.gov.in. Under Login as, choose Beneficiary, not Operator. Type your Mobile Number and the Captcha, tap VERIFY, enter the OTP, then tap LOGIN.
  • What you need. A mobile number that can receive that OTP, your Aadhaar number, and a ration card or family ID if you must search another way.
  • What the three messages mean. Your card is getting processed, please wait for some time, thank you for your patience means the card is not ready to download. Wait. Do not keep resubmitting e-KYC. e-KYC is completed and record is sent for approval. means a state official still has to clear it. That is not a refusal. No records found linked to your Aadhar Number means the source list does not hold you under that Aadhaar. Search on a second field before you assume you are out.
  • If the desk said you are not eligible, or the card stays pending. Search on a different field, then try an Arogya Mitra desk or a Common Service Centre. If that fails, register the grievance at cgrms.pmjay.gov.in or call 14555.

West Bengal implemented AB PM-JAY on 8 June 2026 as the 36th and last State or Union Territory. Use the same national portal. pmjay-sha.wb.gov.in is a restricted staff login, not a citizen status page.

The National Health Authority runs PM-JAY. Each State Health Agency holds the state list. ABHA is a separate 14-digit health-record number, not an Ayushman card. Use The RTI Playbook if your name is wrongly missing.

Step-by-step

You can do this on a phone. The portal will not ask you to pay.

  1. Open beneficiary.nha.gov.in. Under Login as, choose Beneficiary, not Operator.
  2. Enter your Mobile Number and the Captcha, then tap VERIFY.
  3. Enter the OTP and tap LOGIN. Tap RESEND OTP if nothing arrives.
  4. On the search screen pick a value in Search By. Fields include Aadhar, Family ID / Ration Card no. and PMJAY ID, with Select State and a rural or urban filter. Your State Health Agency sets the list.
  5. Tap SEARCH. Your family appears under Beneficiary Search Result with a Card Status for each member.
  6. Download the card if it is ready. If it is getting processed, check back later rather than redoing the e-KYC.
  7. For treatment, go to the Arogya Mitra desk at an empanelled hospital, shown on the portal as PMAM. It verifies you and raises pre-authorisation.
  8. After discharge, ask for the claim reference in writing. The hospital files the claim and the State Health Agency settles it.

If the hospital desk says you are not eligible

If you are standing at the counter and someone says you are not eligible, treat it as a search problem first. This is the commonest failure at the desk, and it is usually not a final decision. Ask the Arogya Mitra to search on a second field, Aadhaar first and then the ration card or family ID. Ask for the pre-authorisation reference if one was raised.

If the desk still refuses cashless care while you are admitted, that is an emergency grievance. The AB PM-JAY Grievance Redressal Guidelines treat denial of treatment at admission, and demands for money for free services, as SOS grievances with a 6 working hour turn-around before the State Grievance Nodal Officer. Register it at cgrms.pmjay.gov.in or through 14555, keep the Unique Grievance Number, and write to the State Health Agency. See cashless denied complaint guide.

7 reasons your status may be stuck

If nothing on the portal moves, match your case to one of these.

  • Your family is not on the source list. PM-JAY needs no enrolment. It reads an entitlement list, so a family that is not on it will never appear.
  • The e-KYC match score is below 80 per cent. The portal compares your Aadhaar e-KYC data with the source record. Above 80 per cent it is auto approved. Below it, the record waits for manual approval.
  • Aadhaar is not linked to a working mobile number, so the OTP never arrives. Update it at a UIDAI enrolment centre. See Aadhaar update status check.
  • The hospital is not empanelled for that treatment. Check its specialities on hem.nha.gov.in/search first.
  • Pre-authorisation was not raised or approved. Ask the Arogya Mitra desk for the reference, then use the grievance route rather than paying.
  • The cover for the year is used up. ₹5 lakh is shared across the family. Confirm the balance first.
  • Treatment in another state. Benefits are portable across India. If a hospital refuses portability, raise a grievance.

Pro tips most don't know

  • ABHA is not the Ayushman card. It is a 14-digit health record number any resident can create free at abha.abdm.gov.in, and it gives no cover by itself.
  • The Arogya Mitra desk is the fastest route. Every empanelled hospital has one. It can search the database, do the e-KYC and raise pre-authorisation while you wait.
  • A Common Service Centre may charge ₹30 and no more. The grievance guidelines treat a demand above that approved cost for an Ayushman card as a registrable grievance.
  • Some states run a converged scheme. Odisha runs PM-JAY with Gopabandhu Jan Arogya Yojana and Delhi adds a top-up, so local cover may exceed the central scheme.
  • Senior citizens aged 70 and above are covered irrespective of income. The portal calls this the AB PM-JAY 70 Plus Senior Citizen Scheme and issues a separate Ayushman Vay Vandana card. See Ayushman Vay Vandana Yojana (70+).

Helpline + contact

PM-JAY runs a toll-free call centre on 14555. For Ayushman Vay Vandana, the Ministry of Health and Family Welfare has said beneficiaries may give a missed call to 1800-110-770 or call 14555. Written queries go to [email protected] or to the National Health Authority, 9th Floor, Tower-1, Jeevan Bharati Building, Connaught Place, New Delhi 110001. State Health Agencies run their own desks.

If the portal or hospital will not explain, file an RTI

If the portal, the hospital desk and the helpline cannot explain a denial, ask the public authority under RTI Act 2005 §6. A reply is due in 30 days under §7(1). The National Health Authority is an attached office of the Ministry of Health and Family Welfare, so it can be asked, and so can your State Health Agency.

ABHA card status check

If you searched for an ABHA card, you are in a different queue. ABHA will not pay a hospital bill.

  1. If you already have one, use Login. If you have forgotten it, use Find ABHA using mobile number.
  2. Otherwise use Create ABHA Number. The result is a 14-digit ABHA number.
  3. ABHA support runs on 1800-11-4477, not on 14555.

PMJAY card download (Ayushman Bharat)

You download the Ayushman card from the same beneficiary login. There is no second website for the PDF.

  1. Log in at beneficiary.nha.gov.in as a Beneficiary.
  2. Search for your family, then read the Card Status against each member.
  3. Download the card when the portal allows it. A card that is getting processed is not ready yet.

If your name is not on the list, ask your State Health Agency which source list applies, because an Aadhaar number alone does not make you eligible. Redo the e-KYC at an Arogya Mitra desk or Common Service Centre if the match score was low. If you should qualify and still do not appear, register a grievance, then file an RTI. Find the hospital before you travel. Citizens search here, not on the hospital-side login.

  1. Open hem.nha.gov.in/search, the Hospital Empanelment Module search.
  2. Use the By Pin Code, By District or By Facility Name tab, or Advanced Search.
  3. Each result shows the Facility ID, type, name, address, contact, specialities and empanelment year.

Do not use hospitals.pmjay.gov.in for this. That site is the hospital-side empanelment login and redirects hospitals to the newer module. PMJAY claim status

The beneficiary portal is for the card. The hospital files the claim, not you.

To track:

  1. Ask the hospital, in writing, for the claim reference generated at admission.
  2. Ask the State Health Agency for the status against that reference.
  3. If neither gives a written status, register a grievance, then file an RTI.

How to check Ayushman Bharat status by mobile number

Follow the login steps above at beneficiary.nha.gov.in, then read the Card Status against each family member. The OTP is sent to the mobile number you enter on that screen.

If your mobile number does not work:

  • Visit a Common Service Centre or empanelled hospital with your Aadhaar. The operator can search by Aadhaar or ration card.
  • Call 14555. The call centre can register the query as a grievance for your district nodal officer.

Ayushman Bharat card not received? Troubleshooting guide

If the card never arrives, work these causes in order.

  • Name or detail mismatch with Aadhaar. The e-KYC match score falls below 80 per cent and the record waits for manual approval. See: PAN-Aadhaar name mismatch KYC fix.
  • Mobile number not linked to Aadhaar, so no OTP arrives. See: Aadhaar update status check.
  • Your family is not on the source list. Ask the State Health Agency which list applies.
  • Aadhaar e-KYC not completed. A Common Service Centre or Arogya Mitra desk can finish it.
  • Your state joined recently. Odisha signed in January 2025, Delhi in April 2025 and West Bengal on 8 June 2026 as the 36th and last State or UT, so those records are newer.
  • A data error in the source record. File an RTI to the State Health Agency for your entry.

What to do:

  1. Search at beneficiary.nha.gov.in using more than one field.
  2. If still not found, visit a Common Service Centre or an Arogya Mitra desk.
  3. Register the grievance at cgrms.pmjay.gov.in and keep the Unique Grievance Number.

What is the difference between PM-JAY and PM-JAY SEHAT?

If you live in Jammu and Kashmir, read this before you assume the central list is the only door.

PM-JAY is the central scheme. It gives ₹5 lakh per family per year for listed secondary and tertiary hospitalisation, with no cap on family size, age or gender, portable across India.

PM-JAY SEHAT was launched on 26 December 2020 for Jammu and Kashmir. It covers all residents of that Union Territory, including households outside the central entitlement list, in insurance mode and in convergence with PM-JAY.

How to check Ayushman Bharat eligibility by state

The central entitlement list comes from the Socio Economic Caste Census of 2011, published in full by the Ministry of Health and Family Welfare.

  • Automatically included: households without shelter, the destitute and those living on alms, manual scavengers, primitive tribal groups, and released bonded labour.
  • Rural deprivation criteria: D1 one room with kucha walls and roof, D2 no adult aged 16 to 59, D3 female headed household with no adult male aged 16 to 59, D4 a disabled member and no able-bodied adult, D5 Scheduled Caste or Scheduled Tribe household, D7 landless household living mainly on manual casual labour.
  • Urban occupational criteria: eleven categories, including rag picker, beggar, domestic worker, street vendor, construction worker, sanitation worker, home-based worker, transport worker, shop worker and mechanic (see BOCW registration).
  • Later expansions: senior citizens aged 70 and above irrespective of income, frontline workers such as ASHAs and Anganwadi workers, and, as described in a June 2026 release, gig and platform workers.

States may widen the list from their own databases. To check yours, search at beneficiary.nha.gov.in or ask your State Health Agency which source list it uses.

How to check Ayushman Bharat status offline (without internet)

No data on the phone does not mean you cannot check.

  1. Call 14555, the toll-free call centre, with your Aadhaar or ration card number to hand.
  2. Visit a Common Service Centre. The operator can search the database and do the e-KYC.
  3. Visit an empanelled hospital's Arogya Mitra desk, which verifies eligibility on the spot.
  4. Contact your State Health Agency. Each runs a helpdesk, and a District Grievance Nodal Officer sits in your district.
  5. Register a grievance offline by letter, telephone, email or fax to the State Health Agency or the National Health Authority, or with the District Grievance Nodal Officer.

State schemes that sit beside PM-JAY

Some states run their own hospital cover alongside PM-JAY or converge the two, so entitlement differs by state even though the portal is the same. Odisha joined as the 34th state in January 2025 and runs PM-JAY with Gopabandhu Jan Arogya Yojana, adding ₹5 lakh for women members. Delhi joined as the 35th State or Union Territory in April 2025, with cards from 10 April and a state top-up. West Bengal became the 36th and last to join, on 8 June 2026, covering about 1.43 crore families.

Frequently asked questions

How to check if I'm eligible without going online?

You do not need the portal for this. Call 14555 with your Aadhaar number. A Common Service Centre or an Arogya Mitra desk can also search the database and complete your e-KYC. See eSHRAM card status for related worker schemes.

Is treatment fully cashless?

The Government describes PM-JAY as completely cashless and paperless at empanelled hospitals for listed packages. Ask the Arogya Mitra desk to confirm your treatment is in that list before admission. What if my hospital demands payment?

Ask for the Arogya Mitra first. If the demand continues while you are admitted, register an SOS grievance at cgrms.pmjay.gov.in or through 14555. The guidelines set a 6 working hour turn-around for that category. Steps: Cashless denied complaint guide.

What treatment does the cover include?

The cover is for secondary and tertiary hospitalisation under the national Health Benefit Package. In February 2025 the Ministry of Health and Family Welfare put that package at 1,961 procedures across 27 specialities, states free to customise. Confirm your procedure with the hospital. Senior citizens 70+, what should they do?

The Union Cabinet approved cover for everyone aged 70 and above irrespective of income on 11 September 2024. Apply at beneficiary.nha.gov.in with Aadhaar. Those in an already covered family get a separate top-up of up to ₹5 lakh. Guide: Ayushman Vay Vandana Yojana (70+).

My name should be on the list but the card is not showing, what do I do?

Search again with a different field, such as the family or ration card number. Then redo the e-KYC at a Common Service Centre or Arogya Mitra desk, because a match score below 80 per cent sends the record for manual approval. Can I check PMJAY status using just my ration card number?

Often yes. After login, the Search By list includes Family ID / Ration Card no. alongside Aadhaar and PMJAY ID. The exact fields depend on your state. See also: Ration card status and Add name to ration card.

How many family members are covered under one Ayushman card?

The Government states there is no cap on family size, age or gender, and the ₹5 lakh per year is shared across the family. Senior citizens aged 70 and above get their own cover, an additional top-up where the family is already covered.
