What the Ayushman card is

The Ayushman card is the beneficiary card under Ayushman Bharat PM-JAY. It lets an eligible family get cashless hospital treatment of up to ₹5 lakh per year at empanelled hospitals. The cover is for secondary and tertiary care, and includes up to 3 days of pre hospitalisation and 15 days of post hospitalisation costs (Press Information Bureau, 23 September 2024).

In 2024 the scheme was widened. From 29 October 2024, all senior citizens aged 70 years and above can get the Ayushman Vay Vandana Card with ₹5 lakh cover, whatever their income. Seniors in families already under PM-JAY get an extra top up cover (PIB, PRID 2082288).

ABHA vs Ayushman PM-JAY card

These are two different things. People often confuse them.

  • ABHA (Ayushman Bharat Health Account) is a 14 digit number that links your health records. Anyone can create it free under the Ayushman Bharat Digital Mission. It is a digital health ID, not a treatment cover (PIB, PRID 2017129). Portal: abha.abdm.gov.in.
  • Ayushman PM-JAY card is the card that gives an eligible family ₹5 lakh hospital cover. It is the treatment entitlement card (PIB, 23 September 2024). Portal: beneficiary.nha.gov.in.

In short, ABHA stores your records, while the PM-JAY card pays for hospital treatment. You can hold both. This page is about the PM-JAY card.

How to download your Ayushman card

  1. Open the official portal beneficiary.nha.gov.in, or install the official Ayushman App from the Google Play Store, published by the National Health Authority.
  2. Enter your registered mobile number and verify with the OTP sent to your phone.
  3. Follow the prompts to search your record, usually by state and an ID such as Aadhaar, ration card or family ID.
  4. Open your record and choose the option to view or download the Ayushman card.

How to check eligibility or status

Use the same official portal or app to check if you are a beneficiary.

  1. Open beneficiary.nha.gov.in or the Ayushman App.
  2. Log in with your registered mobile number and OTP.
  3. Use the eligibility or search option to see if your name is in the list.
  4. You can also call the toll free helpline 14555, or give a missed call to 1800110770, to check (PIB, PRID 2082288).

How to find empanelled hospitals

Use the official hospital search.

  1. Select your State and District.
  2. Filter by Hospital Type (Public, Private or GOI) and by Speciality if needed.
  3. You can also search by Hospital Name and set a distance in kilometres.
  4. Open a hospital to see the specialities it covers.

As on 28 February 2026, the scheme listed 36,229 empanelled hospitals, made up of 19,483 public and 16,746 private hospitals (PIB, PRID 2243769). Always confirm the hospital is currently empanelled before you go.

Who is eligible

PM-JAY covers eligible poor and vulnerable families, with a goal of covering over 12 crore families, about 55 crore people (PIB, 23 September 2024). From 29 October 2024, all citizens aged 70 years and above are also covered through the Ayushman Vay Vandana Card, whatever their income (PIB, PRID 2079021). The simplest way to know your status is to check on the official portal or app, or call 14555.

If OTP does not come or name is not found

If the OTP does not arrive

  1. Confirm you typed the mobile number linked to your record.
  2. Check your network and wait a minute, then use the resend option.
  3. Try the official Ayushman App if the website is slow.
  4. If it still fails, call the helpline 14555 for help.

If your name is not found

  1. Try a different ID, such as ration card or family ID, if the portal allows.
  2. Visit the nearest empanelled hospital or an Ayushman help desk, which can also help with registration (PIB, PRID 2082288).
  3. Call 14555 to ask why your name is not showing.

If a hospital refuses treatment

The official position is clear. Empanelled hospitals cannot deny treatment to eligible beneficiaries (PIB, PRID 2243769). If an empanelled hospital refuses, or asks you to pay for covered treatment:

  1. Note the hospital name, date and the name of the person who refused.
  2. Raise the matter through the official grievance system below, or call 14555.
  3. You can use the three tier grievance system at district, state and national level (PIB, PRID 2099547).

RTI angle for denial and unclear eligibility

  • The list of grievances filed and resolved for your district, if held by the office.
