Ayushman Bharat Eligibility & Fraud Guide — Verify Card, Block Misuse (2026)

Ayushman Bharat — Pradhan Mantri Jan Arogya Yojana (PM-JAY) — is the world's largest publicly-funded health insurance scheme, offering ₹5 lakh per family per year. Two scams ride on top: agents charging “card-issuing fees” for what is always free, and empanelled hospitals running phantom admissions / inflated bills against your card without your knowledge. This page is the verification + misuse-detection + escalation playbook.

Citizen Crisis Response Network — Ayushman rules
The card is always free. Verification is via the PM-JAY beneficiary portal + Ayushman App + the NHA helpline 14555. Empanelled hospitals can never charge a beneficiary directly under the scheme.

To verify Ayushman Bharat eligibility and check for misuse: (1) check eligibility at beneficiary.nha.gov.in (PM-JAY) using mobile / Aadhaar / ration card, (2) download the Ayushman App to view all transactions logged against your card, (3) call 14555 (NHA helpline) for any discrepancy, (4) report fake card-issuance agents + hospital fraud at pmjay.gov.in → “Anti-Fraud Cell” or via the State Health Agency, (5) for refused empanelled-hospital service, escalate within 24 hours to the District Implementation Unit (DIU) and State Health Agency. The card and treatment are always free under PMJAY.

In this guide

What Ayushman Bharat covers

  • ₹5 lakh per family per year, top-up over base policies
  • No cap on family size under PM-JAY (since 2018)
  • Pre-existing conditions covered from day one
  • 3-day pre-hospitalisation + 15-day post-hospitalisation expenses included
  • 1,949+ procedures under empanelled rates (cardiology, oncology, transplants, ICU, etc.)
  • 24,000+ empanelled hospitals across India (public + private)
  • Senior citizens — Vay Vandana / 70+ extension (2024) covers all citizens 70+ regardless of family income
  • Portability — card valid in any empanelled hospital across states

The 70+ extension (Ayushman Vay Vandana) is the most under-claimed expansion of 2024-25 — millions of senior citizens haven't yet activated their universal cover.

Check eligibility in 60 seconds

  1. PM-JAY beneficiary portalbeneficiary.nha.gov.in
    • Login → State + scheme → Search by mobile / Aadhaar / ration card / name
    • Result: eligibility status + family member list
  2. Ayushman App (Play Store / App Store — published by National Health Authority)
    • Aadhaar OTP login
    • View card / family members / utilisation history
  3. 70+ Vay Vandana eligibility — separate enrolment for senior citizens regardless of socio-economic category
  4. Helpline14555 (toll-free, 24×7)
  5. Common Service Centre (CSC) — every CSC offers free PMJAY enrolment

If the result says “Not eligible” but you believe you should be — file a grievance at NHA or visit a CSC for re-verification.

Get the card — the only legitimate routes

  • Ayushman App — Aadhaar-based eKYC; instant download
  • CSC (Common Service Centre) — free physical enrolment with biometric KYC
  • Empanelled hospital's Ayushman Mitra desk — kiosk at every empanelled hospital
  • State health camps during Ayushman Pakhwada / Aapke Dwar Ayushman drives
  • District Implementation Unit (DIU) — for grievances or special cases

Cost: ₹0. Anyone charging is committing fraud.

Spot a fake-card scam

Red flag Real Fake
Fee asked ₹0 ₹50 / ₹250 / ₹500
Channel Ayushman App / CSC / hospital “Camp” / “agent” / WhatsApp
Verification on portal Yes No
Card material Standard NHA template, QR + family ID Lookalike laminated print
Promised “extra cover” None “Up to ₹10 lakh / extra hospitals”
Citizen tip — If anyone asks for money to issue an Ayushman Bharat card, report them at 14555 + the State Health Agency. The fraud is sometimes layered with KYC harvesting (your Aadhaar + biometrics get scraped).

Detect hospital misuse against your card

A growing problem: empanelled hospitals book phantom admissions or inflated treatments against your Ayushman ID without your knowledge. The misuse appears as:

  • Treatment in your name on a date you weren't admitted
  • Procedures billed that you never received
  • Family member's admission you weren't told about
  • Repeated admissions consuming your annual ₹5 lakh limit

How to audit

  1. Open the Ayushman App → Utilisation history
  2. Cross-check every entry against family events
  3. For any entry you don't recognise, note: date, hospital, procedure code, claim amount
  4. Call 14555 with the entries
  5. Email the State Health Agency (each state's SHA has its own grievance email)
  6. The NHA's National Anti-Fraud Unit investigates and can de-empanel hospitals + recover funds

30-minute drill if a hospital denies cashless

If an empanelled hospital refuses to admit you under Ayushman:

  1. Verify hospital empanelment — search at hospitals.pmjay.gov.in
  2. Demand written denial with reason
  3. Call 14555 from the hospital reception
  4. Email the State Health Agency (find at pmjay.gov.in → Implementation → State SHAs)
  5. Tweet / public-platform escalation to the State Health Minister + CMO sometimes accelerates
  6. Approach the District Implementation Unit (DIU) — 24-hour SLA
  7. NHRC — for life-critical refusal (Article 21 + right to health)
  8. Local Health & Family Welfare officer
  9. If life-critical and stalled — admit, pay, claim reimbursement; PMJAY rules cover post-facto reimbursement in genuine emergencies

What not to do

  • Do not pay any “fee” / “donation” for an Ayushman card.
  • Do not share your Aadhaar OTP with a “card-camp agent” — this enables silent identity SIMs / loan fraud.
  • Do not sign blank consent forms at hospital admission — read them; some are pre-printed to authorise broader procedures than needed.
  • Do not ignore the utilisation history — your annual ₹5 lakh limit can be silently exhausted by hospital fraud.
  • Do not assume an empanelled hospital can charge you “small extra” — empanelment means zero out-of-pocket for covered procedures.

FAQ

I lost my physical Ayushman card. What now?

The card is not the entitlement — it's a representation. Re-download via Ayushman App with Aadhaar OTP, or visit any CSC / empanelled hospital's Mitra desk.

Can I use Ayushman + ECHS / CGHS / RGHS together?

Generally yes — Ayushman is the “last-resort” cover. State schemes / employer schemes apply first; Ayushman tops up.

Are mental-health treatments covered?

Yes — IPD mental-health treatment is part of the procedure list (post-2018 mental health parity).

Will my data be shared with anyone?

NHA shares only with the empanelled hospital for the relevant claim. DPDP Act 2023 + ABDM rules govern broader data.

Can a hospital refuse Ayushman because "the IT system is down"?

No — IT downtime is an internal issue. Hospital must admit and submit the claim retrospectively. Document the refusal.

Myth vs reality

Myth Reality
“Ayushman card costs ₹250-₹500 from agent.” Always free. Agents charging are fraudsters.
“Only BPL families are covered.” SECC + state expansions + 70+ Vay Vandana cover most non-BPL families too.
“Card valid only in your state.” Portable across India.
“Hospitals can charge a small extra.” No — empanelled = zero out-of-pocket for covered procedures.
“Pre-existing conditions excluded.” All PEDs covered from day one under PMJAY.

Last word

Ayushman Bharat is one of the largest entitlements in Indian welfare history — and yet underused because of confusion about eligibility and misuse by intermediaries. Two minutes on the beneficiary portal + Ayushman App + 14555 answers almost every question. Audit your utilisation history monthly; report any unknown entry. The scheme works best when citizens treat it as their own audit responsibility, not the government's alone.
