Passport Annexure D for a Minor: Current Official Form

Annexure D is Passport Seva's declaration by a minor applicant's parent or parents or legal guardian. The current official PDF says it is on plain paper. It is not required to be notarised or printed on ₹10 stamp paper merely because it is Annexure D. Download the latest specimen from passportindia.gov.in and use the version current on your appointment date. Complete the minor's name and date of birth and the parent or guardian details exactly as in the passport application and identity records. The current form has signature lines for each parent or legal guardian and lets each signatory give a passport number (preferred), Aadhaar, voter ID, PAN or driving licence number.

Annexure D is used in normal minor-passport consent situations, but Passport Seva has separate rules for one-parent non-consent, custody, parents abroad and other special cases. 1. Open Passport Seva's Annexures page and download Annexure D directly from the official domain.
2. Print it on plain paper. Do not add stamp paper or notarisation unless a separate, case-specific official requirement applies.
3. Enter the minor's name and date of birth exactly as in the online application and birth proof.
4. Complete each parent or guardian section with the identity details accepted in the current specimen and sign in the indicated places.
5. If a legal guardian signs, carry the court order appointing the legal guardian so your authority can be verified.
6. Carry the appointment receipt and original supporting documents selected by the official Document Advisor. For one-parent non-consent or custody issues, read the special-case route first.
