Direct answer. Death must be registered within 21 days at municipality where death occurred (RBD Act 1969 §13). Hospital usually files Form 4 (cause of death) automatically; family then files Form 2 (registration). For deaths outside hospital: doctor's certificate + family affidavit. Free within 21 days; nominal late fee.

Mandatory documents

  • Hospital discharge slip / Form 4 (cause of death) — For hospital deaths — issued by treating hospital
  • Doctor's death certificate (Form 4A) — For deaths outside hospital — by registered medical practitioner
  • Aadhaar of deceased — Identity
  • Aadhaar of informant (next of kin) — Person filing application
  • Form 2 (Death Registration Application) — Standard form; available at registrar
