Quick summary
The Unique Disability ID (UDID) is a single, nationally recognised card for persons with disabilities (दिव्यांग). It is issued through the swavlambancard.gov.in portal by the DEPwD.
The disability certificate records your assessed condition. The UDID card carries this information in one document accepted across India.

Go to swavlambancard.gov.in in your browser. Check that the web address ends in gov.in before you enter any details.
On the homepage, click the “Track Application Status” option. This opens the tracking form on the track page.
Type your enrollment / application number. You may instead use your Aadhaar number if it was linked during registration.
Enter your date of birth and the captcha code shown on screen. Take care to copy the captcha exactly.
Click “Submit”. The portal shows your current stage, such as pending verification, sent for medical assessment, or card generated.
You can also install the official UDID app, choose “PwD Login”, and enter your enrollment number and date of birth to view the same status.

First, recheck your status after a few days, as medical assessment can take time. Confirm your registered mobile number is correct so you receive updates.

Only swavlambancard.gov.in is the official portal of the Department of Empowerment of Persons with Disabilities. Use its “Track Application Status” option. Other sites that ask for payment to check status are not official and should be avoided.
Your enrollment or application number appears on the acknowledgement shown when you applied, and is often sent to your registered mobile. If you linked Aadhaar during registration, you may be able to track using your Aadhaar number and date of birth instead.
