What the form needs
- Appellant name + complete address
- Original RTI application no. + date
- PIO's reply date + content summary
- FAA's decision date + content summary
- Specific grounds for second appeal
- Reliefs sought
- Affidavit confirming the facts (notarised on Rs 10 stamp paper)
- List of enclosures (RTI, PIO reply, FAA order, payment proof)

Tips + common mistakes
- 90-day window from FAA decision
- Cite specific case-law (CIC decisions, SC rulings)
- For state RTIs use State IC, NOT CIC
- Penalty under §20 — file separately if PIO delayed unjustifiably
If submitted form is rejected / delayed — file an RTI
