• Proof of Address — Aadhaar · Voter ID · Driving Licence · Bank passbook (with address) · Telephone/Mobile bill (≤1 year) · Electricity bill · Water bill · Gas connection bill · Income tax assessment order · Spouse's passport (with same address)
• Proof of Date of Birth — Birth certificate · Matriculation/SSLC certificate (with DOB) · Aadhaar · PAN · Driving Licence · Service photo ID issued by central/state govt
• Annexure forms — Annexure E (standard affidavit on plain paper) · Annexure D (parents' declaration for a minor; Annexure C when one parent has not consented) · Annexure F (lost/damaged passport only) — download from passportindia.gov.in
• Photographs — not required at PSK/POPSK (photo is captured there); only DPC/SPC/CSC applications need one 4.5 × 3.5 cm colour photo, plain white background

• Marriage certificate — For surname change post-marriage
• Divorce decree — For divorced applicants reverting to maiden name
• Annexure J — Joint declaration by husband and wife, accepted in lieu of a marriage certificate
• Old passport — For re-issue applications — original + photocopy of first 2 + last 2 pages
• NOC from employer — Govt/PSU/statutory-body employees: No Objection Certificate (Annexure G) or Prior Intimation letter (Annexure H)

• Aadhaar + Voter ID + Driving Licence — 3 popular IDs that satisfy PoI + PoA + DOB simultaneously.
• Tatkal route: same address + date-of-birth proof + any 3 of the “out of turn” documents (any 2 for under-18s); no proof of urgency needed, police verification happens after issue.
• Children under 18 — only parents' documents + birth certificate. School ID acceptable as PoI.
• Documents must be originals for PSK verification (returned). Self-attested photocopies for file.
• Address proof must be current — no old utility bills accepted.
• Photo specifications (DPC/SPC/CSC only): colour, plain white background, recent photo that fits the box — no uniform.
