Quick answer: Open beneficiary.nha.gov.in, log in with your mobile number and OTP, then search by Aadhaar, name plus state, or ration card. If eligible, download the e-card. Cover is ₹5 lakh per family per year for listed hospital care. Call 14555 if you cannot log in.
Check the official NHA beneficiary portal first. Do not pay an agent. The National Health Authority runs PM-JAY; State Health Agencies run the state database. ABHA is a separate health-record ID. Use The RTI Playbook if your name is wrongly missing.

Dr. Shrawan Kumar Pathak took his father to an empanelled hospital. The desk said the family was “not eligible” even though they believed they were on the SECC list. His daughter Kashvi Pathak asked the Pradhan Mantri Aarogya Mitra to search again on beneficiary.nha.gov.in with Aadhaar and ration card, then asked the hospital for the pre-auth reference. If the desk still refuses cashless care, file a written complaint with the State Health Agency and an RTI under §6 asking for eligibility status, the pre-auth record, and the reason for denial. See cashless denied complaint guide.

If the portal, hospital desk or helpline cannot explain a denial, you have a statutory right under RTI Act 2005 §6 to ask the public authority. A reply is due in 30 days under §7(1).

For pmjay vs pmjay sehcat, pm jay vs sehcat, difference between pmjay schemes:
PM-JAY (Pradhan Mantri Jan Arogya Yojana) is the central scheme. It provides ₹5 lakh per family per year for listed secondary and tertiary hospitalisation. The National Health Authority publishes current beneficiary counts on nha.gov.in and pmjay.gov.in — use those dashboards, not a private website.
PM-JAY SEHAT (Social Health Insurance Scheme for All) was launched in December 2020. It extends PM-JAY-style coverage to all residents of Jammu & Kashmir, including those not in the SECC list. While PM-JAY is for SECC-identified poor across India, SEHAT covers the universal population of JK.
Key differences:
| Feature | PM-JAY | PM-JAY SEHAT (JK) |
| Launch | September 2018 | December 2020 |
| Coverage | SECC-identified families (see NHA dashboard) | All JK residents |
| Sum insured | Rs. 5 lakh/family/year | Rs. 5 lakh/family/year |
| Eligibility base | SECC 2011 deprivation criteria | Universal (JK residents) |
| Administrator | National Health Authority | JK State Health Agency |
| Portability | Pan-India | Pan-India (via NHA) |
State variants: Several states run their own convergent schemes — e.g., Mukhyamantri Jan Arogya Yojana (Maharashtra), Tamil Nadu Chief Minister's Comprehensive Health Insurance, and Rajasthan health scheme. Check your state's specific scheme: Rajasthan health scheme status.
PM-JAY eligibility varies by state because each State Health Authority (SHA) manages its own beneficiary database. While the core eligibility comes from SECC 2011, many states have added their own categories:
To check your state-specific eligibility:

Not everyone has internet access. You can check your PMJAY eligibility offline:
Some States run their own hospital-cover schemes alongside or instead of the central PM-JAY list. Eligibility can therefore differ by State even when the NHA portal is used. Check pmjay.gov.in for your State Health Agency, and verify the hospital on hospitals.pmjay.gov.in before admission. Related: Rajasthan health scheme status and health insurance schemes in India.
Call 14555 with your Aadhaar number — they'll check and confirm. You can also visit any Common Service Centre (CSC) or the PMAM desk at an empanelled hospital. See eSHRAM card status for related worker schemes.
Yes for empanelled hospitals on listed packages. Some non-package items (specific imports, super-luxury room) may have co-pay. Always confirm with the hospital's PMAM before admission. See Cashless denied guide and Ayushman cashless denied complaint.
Refuse + insist on PMAM contact. If they still insist, file complaint with SHA + RTI for hospital empanelment terms. Detailed steps: Cashless denied complaint guide.
OPD generally NOT covered. Dental only for accident-related, not routine. Mental health limited. See Health insurance schemes India.
Universal coverage announced 2024. Apply at beneficiary.nha.gov.in with Aadhaar; no SECC requirement. Full guide: Ayushman Vay Vandana Yojana (70+).
This is a common data-linking issue. Try searching with different name spellings, your father's name, or ration card number. Visit the nearest CSC or PMAM desk for biometric verification. If still not resolved, file an RTI to your State Health Authority asking for your SECC deprivation category and PMJAY seeding status. See: RTI guide and RTI for scheme delay.
Yes. At beneficiary.nha.gov.in, select “Ration Card Number” as your search criteria and enter your ration card number with the correct state. The portal will show all eligible family members linked to that card. See also: Ration card status and Add name to ration card.
PM-JAY covers the entire family as a unit — there is no cap on the number of members. The Rs. 5 lakh per year is shared across all family members on the same card. Eligibility is determined at the family level based on SECC 2011 data. Senior citizens 70+ get their own individual coverage regardless of family status.
To check status: use the official portal listed above with your reference number. SLA is published; if stuck beyond it, file a free RTI.
