Ayushman Bharat — Pradhan Mantri Jan Arogya Yojana (PM-JAY) — is the world's largest publicly-funded health insurance scheme, offering ₹5 lakh per family per year. Two scams ride on top: agents charging “card-issuing fees” for what is always free, and empanelled hospitals running phantom admissions / inflated bills against your card without your knowledge. The card is always free. Verification is via the PM-JAY beneficiary portal + Ayushman App + the NHA helpline 14555. Empanelled hospitals can never charge a beneficiary directly under the scheme.
To verify Ayushman Bharat eligibility and check for misuse: (1) check eligibility at beneficiary.nha.gov.in (PM-JAY) using mobile / Aadhaar / ration card, (2) download the Ayushman App to view all transactions logged against your card, (3) call 14555 (NHA helpline) for any discrepancy, (4) report fake card-issuance agents + hospital fraud at pmjay.gov.in → “Anti-Fraud Cell” or via the State Health Agency, (5) for refused empanelled-hospital service, escalate within 24 hours to the District Implementation Unit (DIU) and State Health Agency. The card and treatment are always free under PMJAY.
The 70+ extension (Ayushman Vay Vandana) is the most under-claimed expansion of 2024-25 — millions of senior citizens haven't yet activated their universal cover.
If the result says “Not eligible” but you believe you should be — file a grievance at NHA or visit a CSC for re-verification.

Citizen tip — If anyone asks for money to issue an Ayushman Bharat card, report them at 14555 + the State Health Agency. The fraud is sometimes layered with KYC harvesting (your Aadhaar + biometrics get scraped).
