Annexure D — Minor Passport Parental Consent (2026)
Mandatory for minors under 18 (joint custody). For single-parent / sole-custody: use Annexure C / G instead.
What the form needs
- Annexure D form (download from passportindia.gov.in)
- Both parents signatures + photos
- Both parents Aadhaar + PAN
- Marriage certificate copy
- Notarised on Rs 10 stamp paper

Submit at PSK with passport application.

Free (passport fee separate).
Timeline
Same as passport: 7-30 days.
Tips + common mistakes
- Both parents must sign (joint custody assumption)
- Sole custody: court order + Annexure C
- Adoption: Annexure G + adoption deed
- Single parent (deceased): Annexure C + death certificate
If submitted form is rejected / delayed — file an RTI
Forms get “lost”, “incomplete”, “wrong section” — RTI brings out the actual file noting and officer name, which usually resolves the issue within 15 days.
